You can check your child’s immunization compliance by accessing ParentVUE.

• Immunization Record: An updated immunization record showing that the child has received the required immunizations. This record must include the dates the immunizations were received including the month, day, and year.
• Immunization Documents:
  • A written statement from a health care provider specifying all administered immunizations.
  • A healthcare provider’s written statement of a permanent or temporary medical exemption.
  • A notarized Religious Exemption Form objecting the administration of immunizations on the grounds this conflicts with his or her religious tenets or practices.

• Families may request their child's immunization from their primary health care provider. Some may charge for copies.
• Families may access their child’s immunization record through the Virginia Immunization Information System at no cost by submitting the  VDH Immunization Record Request Form .
• If a student was immunized outside of Virginia, families may request records from other states’ immunization record systems which can be found on the CDC Immunization Information Systems (IIS) page.

Parents/guardians are able to add immunization information to ParentVUE using the Online Verification/Update (OVU) Packet. Below are the steps to submit documentation via ParentVUE. 

1. Log in to your ParentVUE account.
2. Select “Online Packets” in the upper right-hand portion of the page.
3. Select “Online Verification” packet.
4. Click through each section to review/enter parent and student information, as necessary, including student health information. 

Once the “Immunization” page appears, scroll to the Immunization Grid.

1. Enter the immunization dates on your child’s immunization record, as needed.
2. Click “Save and Continue.” The “Documents” page will appear.
3. You may either upload your student’s immunization record OR click the checkbox to bring your student’s immunization record to the school or click “Save and Continue.” The “Review” page will appear.
4. Scroll through to ensure the data entered is correct.
5. Click “Submit.”

You may also contact the front office at your child's school to hand deliver their immunizations records. Due to the sensitivity of this health information, families should not share immunization records via email.

Note: Other Immunization Documents (a Religious Exemption Form, a written statement from a health care provider of a permanent or temporary medical exemption, or a written statement from a health care provider specifying all administered immunizations) can be submitted via email or hand delivery. Please see FAQ titled “How Can I Submit My Child’s Immunization Documents” below.

How can I submit my child’s immunization documents (including a Religious Exemption Form, a written statement from a health care provider of a permanent or temporary medical exemption, or a written statement from a health care provider specifying all administered immunizations)? 

• Families should reach out to their school’s Student Information Assistant (SIA) or Registrar to submit these documents. Please contact the front office at your child's school to connect with these staff directly. 
  • Per Regulation 2101, medical exemption documentation must include: 
    • The specific contraindicated immunization(s) 
    • Specification of a temporary or permanent medical exemption. Temporary medical exemptions must include an expiration date 
    • A signature of the health care provider 
    • Name and contact information of the health care provider 
  • Per Regulation 2101, religious exemption documentation must include: 
    • A notarized Religious Exemption Form objecting the administration of immunizations on the grounds this conflicts with his or her religious tenets or practices.
• These Immunization Documents cannot be uploaded to ParentVUE but they may be submitted via email or hand delivery. 
• Note: Immunization Records can be submitted through ParentVUE. Please see FAQ titled “How Can I Submit My Child’s Immunization Record” above.

Due to the sensitivity of this health information, families should not share immunization documentation via email.

Families should reach out to their school’s Student Information Assistant or Registrar to verify their child’s records. Please contact the front office at your child's school to connect with these staff directly.
